Forms for Medication

Medications cannot be accepted without the supporting medical paperwork. 

Due on your camper’s first day of camp

If submitting ASTHMA Medications
Click on each form, print out both, and hand in completed forms with the medication on the first day of camp to the camp nurse

• Asthma Treatment Plan form must be filled out by both parent/guardian and physician 
• Medication Authorization must be filled out by both parent/guardian and physician

If submitting ALLERGY Medications
Click on each form, print out all 3, and hand in with the medication on the first day of camp to the camp nurse

• Food Allergy & Anaphylaxis Emergency Care Plan form must be filled out by both parent/guardian and physician
• Designee Consent form must be filled out by parent/guardian
• Medication Authorization must be filled out by both parent/guardian and physician

If submitting OTC/OTHER Medications
Click on the form, print out, and hand in with the medication on the first day of camp to the camp nurse
