Notify the school nurse if your child has any allergies. If your child has a Life Threatening Allergy and requires an Epi Pen, please have your doctor complete an Allergy Action plan/Epi-Pen Form. This form must must be renewed every school year. See "Forms" below

New Jersey requires students with asthma to have an Asthma Action Treatment Plan on file with the School Nurse. It must be completed by you and your child’s doctor and provided to your school nurse. This document needs to be renewed every school year. See "Forms" below

If your child should need medication in school, parent/guardian and the physician must complete an authorization form. NO MEDICATION, INCLUDING OVER THE COUNTER MEDICATION, WILL BE GIVEN WITHOUT A COMPLETED MEDICATION AUTHORIZATION FORM. See "Forms" below
