Medications cannot be accepted without the supporting medical paperwork.

Due on your camper’s first day of camp

Click on each form, print out both, and hand in completed forms with the medication on the first day of camp to the camp nurse
- Asthma Treatment Plan form must be filled out by both parent/guardian and physician
- Medication Authorization must be filled out by both parent/guardian and physician

- Food Allergy & Anaphylaxis Emergency Care Plan form must be filled out by both parent/guardian and physician
- Designee Consent form must be filled out by parent/guardian
- Medication Authorization must be filled out by both parent/guardian and physician
